Case Studies: Our Approach in Action

1. Chronic Lower Back Pain

Presentation: 48 year-old male with ongoing stiffness, pain with bending, difficulty sitting for long periods.

 Our Approach: 

Physio: This client visited our physiotherapist. He was assessed and identified reduced joint mobility over the L3-L5 levels. This is due to the ongoing long hour sitting the reduces the length of the multifidus muscle. There were some neurological signs with numbness in his leg leg. The physio further assessed and realised a L5 nerve root irritation which causes radicular pain in the back and lower limb. 

 

Joint mobilisation, electrotherapy, dry needling, shockwave is provided which shows improvement in symptoms. Referral to exercise physiologist and remedial massage therapists are provided. 

Exercise Physiology: As symptoms improves, core and hip strength program delivered in our in house pilates studio along with a home exercise program. This client chose to carry out the exercise program with supervision of our exercise physiologist. 

 

Remedial Massage: Soft tissue release to reduce muscle guarding. 

 Outcome: Improved mobility, resolved pain, returned to full work duties. 

2. Achilles Tendinopathy

Presentation: 32-year-old recreational soccer player with a 6-month history of pain at the back of the heel, worse with the first steps in the morning and sprinting during training. 

 Our Approach: 

Podiatry: A thorough foot and ankle biomechanical assessment identified overpronation and reduced ankle dorsiflexion contributing to excess strain on the Achilles tendon. Custom orthotics were prescribed to improve load distribution, and footwear recommendations were provided. 

Physio: Diagnostic testing confirmed mid‑portion tendinopathy. The physiotherapist administered shockwave therapy across multiple sessions, performed soft tissue release of the calf complex, and implemented a structured loading program beginning with isometrics and progressing into eccentric strengthening. 

Exercise Physiology: As symptoms stabilised, EP guided the client through a progressive strengthening plan focused on calf, foot intrinsic muscles, hamstring, and hip control. A staged return‑to‑running protocol was introduced, with monitoring to ensure load tolerance. 

Remedial Massage: Myofascial release to the gastrocnemius–soleus complex to reduce tightness and improve mobility.

Outcome: The client achieved significant reduction in morning pain, improved tendon load tolerance, and returned to soccer training and competition without flare‑ups. 

3. Shoulder Impingement

Presentation: 27-year-old swimmer experiencing sharp pain when lifting the arm overhead, difficulty reaching behind the back, and night pain when lying on the affected shoulder. 

 Our Approach: 

Physio: Assessment identified reduced scapular control, rotator cuff weakness, and irritation of the supraspinatus tendon. Treatment included shoulder joint mobilisation (posterior and inferior glides), targeted dry needling to the rotator cuff and upper trapezius, and specific strengthening for rotator cuff activation and scapular stabilisation. 

No other profession is involved given the simplicity of injury and recovery. 

Outcome: Pain with overhead movement resolved, shoulder strength significantly improved, sleep normalised, and the client returned to full swim training pain‑free.
