Lion Rocker Health – Terms & Conditions
Welcome to Lion Rocker Health. These Terms and Conditions govern the use of our services, including physiotherapy, remedial massage, and other allied health treatments. By booking or attending an appointment, you agree to be bound by the terms outlined below.
1. Appointments and Cancellations
- Clients are expected to arrive on time for all scheduled appointments. Late arrivals may result in a reduced consultation time or rescheduling at the discretion of the practitioner.
- A minimum of 12 hours’ notice is required for cancellations or rescheduling of standard appointments.
- For appointments booked under NDIS, Workers’ Compensation, or other third-party arrangements, up to 48 hours’ notice may be required due to the administrative and scheduling complexity involved.
- Failure to provide adequate notice may result in a cancellation fee, which may be up to the full cost of the scheduled appointment.
- Repeated missed or cancelled appointments may result in restricted booking privileges or discharge from care.
2. Fees and Payments
- All consultation fees are payable at the time of service, unless prior arrangements have been made.
- Payments can be made via EFTPOS, credit card, HICAPS (for eligible private health fund claims), or bank transfer.
- If a client chooses to claim through private health insurance, Medicare, or a third-party insurer (e.g., NDIS, Workers’ Compensation), it is their responsibility to ensure their eligibility and coverage.
- In the event that a third-party funder declines or only partially covers the claim, the client remains personally liable for the full balance of the service fee.
Any outstanding amounts must be paid within 7 days of the invoice date unless otherwise agreed in writing.
3. Health Fund Rebates and Medicare
- Lion Rocker Health provides on-the-spot claims processing via HICAPS for eligible private health insurers.
- Medicare rebates may apply to eligible services if you have a valid referral and Enhanced Primary Care (EPC) plan from your GP.
- It is the client’s responsibility to verify eligibility and entitlements with their health fund or Medicare.
- Declined or reduced rebates are the responsibility of the client, and the outstanding balance must be paid in full.
4. Clinical Records and Privacy
- Lion Rocker Health complies with the Privacy Act 1988 (Cth) and the Australian Privacy Principles regarding the collection, storage, and use of personal and health information.
- Your personal information is used solely for the purpose of delivering safe and effective care.
- Clients may request access to their records in writing. A reasonable fee may apply for retrieval or copies of records.
5. Consent to Treatment
- By attending an appointment, you provide informed consent to receive treatment deemed appropriate by your clinician.
- You have the right to ask questions, refuse, or withdraw consent at any stage.
- While we strive to deliver the highest standard of care, treatment outcomes vary and no guarantees are made regarding effectiveness.
6. Duty of Disclosure
- Clients must provide accurate and complete health and personal history relevant to their treatment.
- Failure to disclose important information may compromise care and result in adverse outcomes, for which Lion Rocker Health cannot be held liable.
7. Code of Conduct
- All staff at Lion Rocker Health adhere to the National Code of Conduct for Health Care Workers (Australia).
- Clients are expected to treat staff and others with respect. Aggressive, abusive, or inappropriate behaviour will not be tolerated and may result in refusal of service.
8. Liability
- While our practitioners strive to provide evidence-based care, all health services carry inherent risks.
- Lion Rocker Health and its staff are not liable for any loss, damage, or injury unless required under consumer law or negligence can be demonstrated.
- Clients are encouraged to follow clinical advice and report any adverse effects promptly.
9. Changes to Terms
- Lion Rocker Health may revise these Terms and Conditions at any time.
- The most current version will be available on our website or at reception. Continued use of our services indicates acceptance of any updates.
